A cased Member of the Most Excellent Order of the British Empire (MBE) medal. The silver gilt medal consists of a cross patonce surmounted by a crown. A medallion at the centre of the cross features Britannia surrounded by the motto "FOR GOD AND THE EMPIRE " The medal is attached to a crimson bow by a simple suspension ring and is held in a square presentation case with " M.B.E." marked on the lid. " GARRARD & Co. LTD / by special appointment / TO HIS MAJESTY THE KING / A4 ALBEMARLE ST LONDON / DALHOUSIE SQ & CALCUTTA " is marked on the silk lining on the underside of the lid.

Ann Burgess was awarded this honour in 1918 as recognition for her work for the welfare of soldiers during World War I. Together with her husband, Mrs Burgess played an active role supporting the community effort during World War I and supported veterans after the war ended. Mrs Burgess formed the Women’s Patriotic Committee that presented parcels to new recruits before they left New Plymouth for war and she organised the first New Plymouth ANZAC Day reunion for ex-servicemen and women and patriotic workers in 1918. From this small beginning the annual gathering grew to draw crowds of over 1000 people until ill health finally forced Mrs Burgess to step down from organising the event in 1947.
